SENIOR COMMUNITY SERVICES WORKER

General Functions
   Performs professional and administrative social work involving complicated and difficult cases. Provides social and re-entry services to county residents through case planning, case management, direct service delivery, and follow up. Based on area of assignment, provides information and referral service, general assistance, counseling, medical assistance, mental health services and discharge planning. Collaborates closely with community providers.

Work is performed under minimal supervision of an assigned supervisor, who evaluates work through observation, file reviews and statistical reports indicating results achieved.
 
Representative Duties /Assignments
   Duties may vary based on area of responsibility.
  • Screens and interviews applicants for social services in compliance with county, division and program policies.
  • Performs case management, discharge planning and re-entry assistance for incarcerated patients on medication assisted treatment.
  • Makes home visits and other investigations to ascertain the exact nature of home environment and to supplement information gained by the personal interview.
  • Makes appropriate referrals and advises clients of available program resources.
  • Attends and conducts periodic team meetings; confers with superiors on difficult assignments; prepares reports and correspondence on assigned cases and maintains related status reports.
  • Assures Program compliance with County, Division, contract and accreditation requirements.
  • Supervises and evaluates program staff as assigned.
  • Monitor Child Comprehensive files as needed.
 
Minimum Qualifications
   Bachelors degree in Social Work, Sociology, Counseling, Psychology, Health or Behavioral Sciences or a closely related field and three years of directly related social service work experience or a Masters degree in Social Work (MSW) and one year of directly related social service work experience.

May be required to possess and maintain a valid Florida Drivers license and provide own vehicle for job related duties.

Must be able to demonstrate intermediate to advanced proficiency in word processing functions, preferably Microsoft Word, Microsoft Excel and email functions.
