| General Functions |
| |
Performs social worker case management to patients under Orange County Secondary Care services. Work is performed under the supervision of the Quality Assurance Specialist and is reviewed and evaluated through observations, file reviews, conferences, reports and results achieved. |

| Representative Duties /Assignments |
| |
- Collaborates with nursing case management to refer patients to medical resources as appropriate.
- Assists patients in social services referrals including Social Security and Medicaid.
- Provides individual and family counseling relating to physician plans of care and healthy decision-making.
- Performs other related duties as required.

| Minimum Qualifications |
| |
Bachelor’s degree in Social Work or Social Sciences including Public Health and three years case management experience in a hospital, social service agency, or nursing home environment.
Case management experience must include assessments, referrals, tracing, monitoring and other interventions as required.
Knowledge of local community services.
Proficient in Microsoft Office.
