| General Functions |

Works as a key member of the Human Resources Benefits Wellness Team to plan and implement comprehensive employee wellness programs designed to identify and promote on-going employee wellness and health lifestyle behaviors. Provides confidential individualized care, counseling, health education, referral and resources to employees.

| Representative Duties /Assignments |
| |
- Assesses County health plan experience, employee HRA and health screening results and related data and information, and identifies wellness needs of the employee population. Works with Benefits and Wellness staff to develop wellness programs, training and initiatives targeted to those needs.
- Conducts individual health risk assessments, and collects individual employee health screening information. Identifies those employees at risk and works with them to develop specific health improvement plans.
- Works closely with Benefits Wellness staffing the planning, organizing, coordination, promotion and implementation of health and County wellness programs and initiatives, to include: Wellness for Life programs, health fairs, wellness lunch n learns, department specific interventions, learning sessions, etc.
- Creates health education curriculum and information for seminars, newsletters, web page articles and other venues.
- Delivers health and wellness training and communication seminars.
- Works with personnel at all levels throughout the County, to include senior management, managers and supervisors, Human Resources Strategy Team members, union representatives, and employees to assure the greatest possible degree of coordination, communication and participation in County Health and Wellness programs.
- Maintains confidential employee health and wellness records, and provides annual follow-ups and records updates.
- Serves as a member of the County Wellness Committee

| Minimum Qualifications |

Graduation from an accredited school of nursing and possession of current license from the Florida State Board of Health as a Registered Nurse. Three (3) years nursing experience to include a minimum of one (1) year as a registered nurse in a public health or community health setting that includes a significant component of wellness or prevention education, counseling and advising. Must possess knowledge of physical assessments and health maintenance protocols. Must be able to work as a team member and/or independently with a minimum of supervision, to deal effectively with management, health providers and to interact and build positive trusted relationships with individual employees. |
